In order to participate in any of our Classes,
Teams, Clinics, or Special Events, we must have a Release & Waiver of Liability
Form on file for each child/adult.

Private lessons are offered Saturdays from 8-10 a.m.
and Sundays from 5-8 p.m. Forms must be submitted and private lessons scheduled
with the instructor by the close of Thursday prior to the weekend of the
scheduled private lesson. No form, no lesson. All checks must be made out to MBGF.
